Please fill out the ENTIRE form. The form cannot be submitted until it is FULLY COMPLETED.
If you have only one book, the payment will be attached to that book. If you have more than one book, YOU MUST FILL OUT A SEPARATE INPUT FORM FOR EACH BOOK, but the FULL payment should be attached to your FIRST book. If you pay by check, one check will cover all of your books.
We will use the same information for both the PRINT version and the PDF version.
PayPal is the preferred method of payment. You do not have to be a member of PayPal to use this service.
After submitting the form, please use PayPal, using the button on the “Review your information” page, or send a check. If the credit card online form contains a memo or comment section, please indicate "CATALOG."
If you cannot use PayPal and need to send a check, the check must be received within 5 days. Please make the check out to PALA and send it to Gary Young, PALA, P.O. Box 8728, Calabasas, CA 91372-8728. Please indicate "CATALOG" on the memo line (bottom, left).
Your listing will become active only upon receipt of payment and verification that you
are a current member of your affiliate.
